    If you are submitting a reimbursement request, please download the attached reimbursement request form (located at the bottom of this page) and attach all receipts. 

    A reimbursement check will be sent to you at the address provided on the form.

    Per SD ENA policies, reimbursement requests must be submitted within 30 days of the event and checks must be deposited within 30 days of receipt.
